Improving Patient Care and Access 2026/27

North H&F GP practices are aiming to improve patient access and care by reviewing how we work, making better use of local services, and listening to residents.

In 2025/26 we increased NHS App registrations, held a number of engagement events, and strengthened partnerships with W12 Together.

Next, we’re continuing to look at improving how we handle calls for faster responses, offering better continuity for vulnerable patients, and helping more people use digital tools to manage their care.
