The Friends & Family Test Have your say to improve your healthcare

Did you know you can now have your say to help improve more services across the NHS?

The Friends & Family Test is about giving patients the
opportunity to provide quick feedback on their care and
treatment experience. It’s in use across most local NHS
healthcare services.
You can say what is going well and what can be improved
so that people who make decisions about local healthcare
can take your views into account.
It doesn’t take long and you don’t need to give your details
on your feedback form.
Next time you visit your GP, dentist or hospital, look for
details in the reception or waiting room or ask a member of
staff.

Starts Monday 18/09/23 1100- 1300

During these 4 sessions course you will learn a variety of relaxation techniques

including visualisation, breathing exercises, and progressive muscular relaxation

techniques to help you relax and be in the present moment.

If you or someone you are with finds themselves in a life-threatening situation call 999 and ask for an ambulance. You will be taken to most appropriate place for your needs which in some case may not be your nearest hospital.

Need to see a doctor? NHS 111 can help

Call 111 and if appropriate, the advisor will be able to schedule you an appointment with a GP – it may not be your usual one – at a local venue. If you could be better treated elsewhere, they will be able to direct you to the best place to suit your needs. 111 is free to call at all times.

Experiencing a mental health problem?

If you are affected by a mental health issues over the bank holidays and are not already known to local mental health services, or call 111. If you are a service user already, you can access services via your Community Mental Health Team or the out of hours GP service.

What to Expect
Get the support you need from our experienced health coaches
around all thing’s behaviour change. Learn how to break unhealthy
habits and make healthy habits stick, to create long-term changes to
your health. With both 1:1 and group sessions available, expand your
knowledge on nutrition, movement, and the relationship between
food and mood

Initial consultation with Health Coach
Healthcare staff will assess your eligibility for the service, and you’ll be
booked in for an initial consultation with a health coach
• 30 minutes getting to know you
• Baseline assessments- BMI, waist circumference, mood, sleep quality,
weekly minutes of activity, alcohol consumption, confidence levels
• Main goals and outcomes discussed
• After the initial discussion, clients will have the opportunity to choose
their preferred 6-week group session or opt for continued personalized 1:1
support
• If the service is not suitable for you, you’ll be directed to the service/
community group that best suits your needs

6- Week Group Session
• Weight Management Pathway or Wellbeing Pathway
• Groups of up to 15 participants
• Weekly educational sessions
• Work as a team with your health coach, focusing on your priorities
• Check ins via e-mail and calls
• Access provided for our online gym and online recipe book

Celebrating 75 years of the NHS

NHS 1948 – 2023

Come and join the Park View Centre for Health and Wellbeing
team in celebrating the 75th anniversary of the NHS on
Wednesday 5 July

Patients and local residents are invited for to commemorate our national health service from 12.00 – 3pm, with guest speakers set to make an appearance during the afternoon.
Everyone is welcome – even if you are not registered with our practice or do not currently have a GP.
If you’d like to volunteer at the event by providing refreshments or soft beverages, please speak to your local practice manager or email
